Senior Reach Connects Residents to Community Resources during Winter Months
*Two out of every three seniors do not receive the care they need*
LAPEER, Mich. - During the winter season, Lapeer Community Mental Health encourages residents to be extra vigilant regarding seniors in their family and community. Four out of five older adults suffer from at least one chronic health condition and one in four experience behavioral health problems. Neighbors, family members and community residents often worry about senior citizens who seem lonely, isolated and without support but may be uncertain how they can help.
Seniors in Lapeer County who are over the age of 60 years of age may be eligible for Senior Reach. Senior Reach is a home-based counseling and case management service program. Senior Reach provides support for the well-being, independence, and dignity of older adults. Services include counseling, referrals to community resources, care management services, and depression screening and treatment.
Senior Reach also trains community members on how to make a referral to Senior Reach. Anyone can contact our call center and make a referral, including the following: employees of agencies, pharmacists, landlords, bank clerks, hair stylists, physician's offices, neighbors and family members. Senior Reach is available to speak with any group who is interested in learning more about identifying the needs of seniors in their lives.
To make a referral, simply call the Senior Reach office and give us the name, address and phone number of the Lapeer County resident (over the age of 60) that may need assistance. Our Senior Reach Specialist will then call and talk to the senior to determine need and willingness for assistance. The referrals can be anonymous and the senior has the right to refuse services. All services are provided in the home of the senior.
Senior Reach is available to any senior who is having some mild to moderate symptoms of depression, anxiety, grief, or loneliness, or who needs some help accessing resources. We cannot presently serve persons living in assisted living or group homes or who have a moderate to severe dementia (although we can help the caregiver who is over 60). There is no income requirement or co- pay. Generous grant funds received from the Health Endowment Fund presently cover the services so there is no charge to the senior. Donations are always accepted.
