Personal A ssistant

My name is Alexis Smith, and I am 23 years old. As a person with a disability I have Homemaker services which is another word for a PA through a program called the "Disability of Human Services" (DHS). As a client of DHS I have 92 hours per month and 23 hours per week. As of right now I have homemakers through an agency called ASI Monday through Sunday, and the agency pays them.

Requirements to becoming a PA

1. you must respond to this ad

2. You must be a responsible female or non-binary individual from the ages of 25 to 45

3. You must have at least a high school diploma as a of requirement from DHS to the state of Illinois.

4. You must be a resident in Chicago preferably near Humboldt Park where I live

5. I would prefer you to have a car, but as long as you have reliable transportation either through the train or bus I will still consider hiring

6. You can not have any physical ailments that would hinder you from completing your assigned task such as diabetes, high blood pressure, arthritis, back, neck, knee, leg, heart or hip problems, asthma, carpal tunnels, and sickle cell anemia

7. You must not be a mother to small children

8. you must not be allergic to cigarette smoke or cats

9. You must be willing to work flexible and sometimes long hours and sometimes be on call

10. You must be active and healthy, take care of yourself

11. If I can find a p a i want them to work the hours of Fridays mostly from 11-1pm or 11-2pm but sometimes from 7:30 a.m. to 9:30 a.m. ( times may change so be flexible)

Saturdays usually from 10 to 12 noon or 11-1pm or 11-2pm ( times may change so be flexible)

Sundays from 11- 2pm ( times may change so be flexible)

12. Be willing to assist me both in the morning and evening for special occasions if needed meaning come to my house twice in one day to help me change my clothing.

13. Be on call to be prepared to work for me if and when my regular Homemakers from ASI don't show up during the week Monday through Thursday. If this happens I will call you at least 24 hours in advance.

14. I would prefer you to have Healthcare experience but it's not necessary

15. You must present me with a resume and references

Duties but not limited to:

1. Making up my bed- change my sheets when necessary

2. Sweeping and mopping my bedroom and bathroom for since it's a common area

3. Take out my bedroom and bathroom garbage

4. Cook food well

5. Help me get dressed meaning help me put on my bra and the rest of my clothing.

6. Help me put on my leg braces, shoes and then tie them.

7. You must know how to style my hair whether it is in its natural state of tight curls or straightened out with a flat iron, you must know how to take down two strand twist

8. Must wash the dishes after I have eating a meal

9. Must assist with grooming( shaving armpit and pubic hair

10. Must clean Furniture in my room

11. Must know how to apply eyeshadow

12. Paint my nails if I want them to be

13. Go to the store for me

14. Attend doctor's appointment with me

15. Go clothes shopping with me

16. Come to my modeling event with me to help me change back into my regular clothing from my performance clothes

17. Take the CTA bus with me if necessary

18. Clean the bath tub after I have showed the night before

19. Stay with me until my Pace Van leaves.

Becoming a PA protocol:

1. Once you have responded to this ad; I will ask you to give me your phone number so that I can have and over the phone interview with you during this time if the conversation goes well we will agree to meet in a public place to discuss things in greater detail. During this meaning I will give you the DHS PA packet for you to fill out if you agreed to work with me.

2. Once it is filled out we will go to the DHS office that is near me to turn it in to the company.

3. The Next Step will be for DHS to put your information into the system as mine PA this will probably take a few weeks.

4. Once you are approved to be my p a i will have you come to my home before your original start date to do a trial run I see how well do things for me within a timely manner.
