The Provider leads a care team that consists of Registered Nurses and Care Coordinators.Job Responsibilities:Delivers cost-effective, high quality care to assigned CareSight patientsCompletes comprehensive history and physical examination on all enrolled patients in an in-home visit.Collaborates with RNs and patients to determine patients' risk of adverse outcomesCompletes routine and unplanned, urgent, and/or emergent in-home visits as needed to meet patients' needs.Responds to calls from patients, caregivers, CareSight RNs, etc. to manage patients' care.Addresses Advance Care Planning proactively; identifies surrogate decision makers.Manages patients' medical care as appropriate including:orders and monitors diagnostic tests including laboratory studies, radiological studies, etc.refers to appropriate specialists or community servicesaddresses recommended preventive and quality measuresmonitors chronic conditions to minimize exacerbations or treat in place if clinically appropriateidentifies, assesses, diagnoses, and treats acute changes of conditionsmanages medication therapy by prescribing medication, adjusting dosages, and administering select medication in the home as clinically appropriate.documents in the electronic medical recordparticipates in clinical case conferences as requiredparticipates in on-call coverage for patient careRequirements:Holds active, unencumbered license to practiceCredentialed in adult or family practiceHolds an active DEA licenseExperience caring for adult, chronically ill patients (2 years preferred)Drivers license without restrictions and good driving recordWorking knowledge of computers and ability to document effectively and efficiently in an electronic systemJob type- full time
